Fix - `ProgramCache::assign_program()` Closed => Loaded in same slot (#673)

* Refactors test_add_builtin() to use LoadedProgram::new_builtin() and ProgramCache::extract().

* Revert Closed => Loaded transition

* Adds more loaded programs deployment tests
