Adding upgrade file watcher for cosmovisor.
Currently the comswisor upgrade mechanism relays on parsing log messages. This is not reliable:
+ depends on the log level output (x/upgrade uses INFO)
+ can be hacked by accidentally logging user user content
+ can be broken by using upgrade name which will break the regex pattern.

The following is a breaking fix to #9800. In the non-breaking fix, the initialization logic was moved out of `InitializeAndSeal` but the API was preserved. I've now removed `InitializeAndSeal` and replaced it with just the `Seal` function, which may be called much more cleanly in `app.go`

## Description
This PR adds a CLI for querying all module accounts, `module-accounts`, in the auth module.
Using this command would display all the account information, including human readable name of the module, module account address, account number, permissions, etc.
This command would be especially useful for developers using cosmos-sdk as there are lots of instances where a developer would have to look into the module account, but only to do so by getting the module account address after getting the module name, hashing it and bech32fying it in the status quo. By using this command, users would be able to get a quick look on the list of all module accounts' information.

This PR is a forward-port of #9392 from `release/v0.42.x` to `master`.
It was initially decided in https://github.com/cosmos/cosmos-sdk/pull/9392#issuecomment-865987655 that #9392 would be a temporary fix for 0.42, but as per @AdityaSripal's request we're actually foward-porting this to master too.

## Description
Related to #9489, this PR improves the UX when using the `withdraw-all-rewards` command by forcing the broadcast mode to `block` if the chunk size is greater than 0. This will ensure that the transactions do not fail even if the user uses invalid broadcast modes for this command (`sync` and `async`).

## Description
Adds the amount of coins slashed from the validator to the `Slash` event. Additionally, before this PR, the jail/slash events were emitted **BEFORE** the slash/jail functions were even ran, which could result in a false positive event if an error occurred in these functions. ~~These events were moved to the end of the functions that implement the logic for them instead.~~ This PR moves the events to be emitted after the logic is executed.

The `--keyring-dir=xxx` flag was allowed by `appd gentx`, but wasn't actually used to search for key parameters. On investigation, it appears that `gentx` only initialized the query-side of the client context, but the tx-side needed to be initialized since keys and signing were involved.
